What a DOT consortium actually costs
Most C/TPAs charge two ways: a fee to be in the program, and a fee per test. The first one is where the money goes. Here's how it works and what you'd pay. A consortium is just the random pool plus the testing program that runs it.
How much does a DOT drug testing consortium cost?
There are two parts to almost every C/TPA's price.
A fee to exist in their system. Called an enrollment fee, membership fee, annual fee, or per-driver fee. As of September 2026, the published range across major providers runs from about $50 to $150 per driver per year, or a flat $99 to $299 a year for the company. You pay it whether a driver ever tests or not.
A fee per test. Published rates run $40 to $90 per DOT urine test depending on the provider, sometimes with add-ons for MRO review, Clearinghouse reporting, or observed collection.
TrueTest charges only the second part. $65 per DOT test. No enrollment fee, no annual fee, no membership fee, no per-driver fee.
What does $65 include?
Collection at any site in the network, laboratory analysis at a SAMHSA-certified lab, and Medical Review Officer review of every result. Random pool management, quarterly selections, Clearinghouse reporting, and audit records are part of the program, not line items.
The one add-on: an observed collection, when the regulation requires one, is $50 more.
DOT breath alcohol tests are also $65.
What does a fleet pay in a year?
Find the example closer to your volume. The counts below are drug tests only. A fleet's drug-test count comes from randoms (50% of drivers a year) plus a pre-employment test for every hire.
Fifteen tests a year
Typical for about 10 drivers with a roster that turns over about once a year, common in this band: 5 random + 10 pre-employment.

Fifty tests a year
Typical for about 30 drivers with a roster that turns over about once a year, common in this band, or a smaller fleet that hires often: 15 random + 35 pre-employment.

Random alcohol tests (10% of drivers a year) are $65 at TrueTest; other providers price them separately, typically $80 or more. There is no pre-employment alcohol test. Post-accident and reasonable-suspicion tests are event-driven and not included.
Different roster? In the per-driver model, add $50 to $150 for every driver on it, tested or not. In the other two, only the test count matters.
The per-driver fee grows with your roster whether anyone tests or not. The per-test difference grows with your hiring. At the cheapest end of the per-driver model the numbers get close; most published rates are not at the cheapest end.
Published ranges as of September 2026. The first two columns are typical pricing structures, not a specific provider; the middle column uses the published member rate of $79.95 per DOT urine test at the lowest-priced consortium we found. Add-on fees for Clearinghouse reporting or observed collections, common in the other two models, are not included. Some providers charge a one-time enrollment fee instead of, or on top of, an annual fee; that is not included.
Do I pay for drivers who never test?
No. A driver on your roster who is never selected and never needs a pre-employment test costs nothing. A fleet that hires ten drivers a year pays for ten pre-employment tests, not ten enrollments.
Why do most C/TPAs charge a fee just to be in the program?
Because a flat fee is easier to forecast than test volume, and most carriers don't compare the two. For a 10-driver fleet, the fee to exist usually costs more than the testing does. TrueTest is a testing company, so we price the testing.
Does the price change if I move my program mid-year?
No. There's no enrollment fee to move, and no gap: your drivers move from one random pool into the other for the next quarterly selection. Here's what to ask your current TPA for before you move.
